Our Services

Personal Support Workers - Outreach Attendant Care

The Outreach Attendant Care service is designed to provide assistance and support to individuals with physical disabilities or chronic illnesses that affect their mobility. Services are offered at home, allowing recipients to maintain their independence and live autonomously in their own environment.

The benefits of this service can include a range of supports and services. Here are some examples:

  • Provide assistance with daily activities such as bathing, dressing, personal grooming, and medication management. 

  • Assistance with moving around the house, getting in and out of bed, and using assistive equipment such as wheelchairs or walkers. 

  • Help with light household tasks, such as preparing light meals, cleaning, and laundry.

  • Social interaction and emotional support to promote the person's mental and emotional well-being.

  • Helps coordinate care with other healthcare professionals and schedule medical appointments.

  • Providing training and education to family members or caregivers to help them take better care of their loved one.

The primary goal of the program is to improve the quality of life for individuals by providing the necessary support so they can continue to live independently and actively participate in their community. The services can vary based on each person's individual needs.

Assisted Living Services for High-Risk Seniors

In support of the Prescott-Russell Community Services, our personal care attendants, under the supervision of our nurse, support eligible service recipients for the program, which is aimed at seniors aged 65 and over living at home in the Hawkesbury, L'Orignal and Vankleek Hill area.

Services are designed to help seniors maintain their independence and quality of life in their own homes or in semi-autonomous community living environments. These services aim to provide support tailored to seniors' individual needs, while enabling them to remain active and engaged in their community.

Here are some of the services available through this program: 

  • This includes help with household chores, meal preparation, etc.

  • Nurse visits for medication management, monitoring of chronic health conditions, general care, etc.

  • Help with getting around the house, the use of mobility aids, and so on.

  • The program offers seniors opportunities to participate in social, recreational and wellness activities such as day programs.

  • Transportation can be coordinated to help people get to medical appointments, run errands or participate in community activities.

  • The team makes itself available to help people cope with social isolation, the loss of loved ones, or other emotional challenges associated with aging.

Support for Caregivers

Our services also include a support structure for caregivers who assist our service recipients. With the informed consent of the service recipient, caregivers can benefit from additional resources to help develop the client's plan of care and can participate in all important discussions related to the service recipient's condition. When our funding allows, we offer respite care options for caregivers.

Support for Access to Essential Equipment and Supplies (this service is only available to our service recipients)

Over the years, we have identified a significant need among our low-income service recipients. Many do not have the financial means to purchase essential equipment or supplies, such as meal replacement products, to meet their needs because they are no longer covered by government programs. Through the Community Fund from Centraide Untited Way, donations of lightly used equipment, and monetary contributions from the community, we have accumulated an inventory of lightly used equipment that we are able to loan without charge to our service recipients.  The funds and donations we receive also allow us to support our service recipients who need adapted transportation or assistance in purchasing nutritious groceries.
